Entry fee: $15 for one event, $25 for two events
   Foundation Members: $10 for one event, $15 for two events.
   Please make checks payable to: Hawaii Pacific Tennis Foundation. Visa or MasterCard also accepted. No refunds after entry deadline.

Please check in at least fifteen (15) minutes prior to your scheduled match.

Doubles players must each submit an entry form.
